All Graduate College forms powered by Frevvo will be initiated by clicking on the form link from the Graduate College forms website or above.
Completing Forms and Determining Workflow
With the exception of the Appointment of Supervisory Committee (ASC) form, Students will determine who the form is routed to by selecting the correct graduate program from a drop down list. Once the student has completed and submitted the form it will be moved to the Graduate Program Coordinator for the program selected above.
For the Appointment of Supervisory Committee Form (ASC) students will be required to enter the contact information for the Chair/Major Advisor serving on their Thesis/Dissertation committee. It is important to verify the accuracy of the contact information provided for the Chair/Major Advisor so that the form is transmitted correctly for approval. Additionally the student will enter the name and contact information for at least two but no more than four additional committee members.
Please Note: At this time students initiating electronic forms will not have the ability to save forms or have forms returned back to them for edits. As those initiating forms are not required to login. Please be sure to use the print button in the top left corner of the form to save a copy of your form prior to submission. Once you form has been fully approved you will receive a copy by email from Graduate Admission and Degree Services.
Workflow Next Steps
The Graduate Program Coordinator may choose to approve or deny a form. If the form is approved it will move forward to the Graduate College for processing and final approval. If a form is disapproved the Graduate Program Coordinator will need to communicate with the student directly and work with them to make corrections or have the student submit a new form. Once the Graduate College has reviewed the form a copy of the form will be returned to the student and Graduate Program Coordinator in either an approved or denied state.
In the case of the Appointment of Supervisory Committee (ASC) form once submitted it will be sent to the Chair/Major Advisor listed. The Chair/Major Advisor will review and approve of the proposed committee. The Chair/Major Advisor may choose to approve or deny this form. If approved it will move forward to the Graduate Program Coordinator for the program selected. The Graduate Program Coordinator will than approve the form and move it forward to the Graduate College for processing. Once the form is approved by the Graduate College an email notification will be sent to the student, Chair/Major Advisor, Additional Committee members for whom contact information was entered, as well as the Graduate Program Coordinator. If an ASC form is disapproved by the Chair/Major Advisor they will need to communicate with the student directly to make corrections or have the student submit a new form.

Accessibility Acknowledgement Statement
The Office of Information Technology, Boise State University, acknowledges that the current version of the software application known as Frevvo Mobile Web Forms Electronic Signatures and Approval Workflows, version 6.2, is not fully accessible for the Spring 2016 implementation by the Graduate College.
Current functionality for those using assistive technology (specifically screen readers) is limited in the following areas:
- The user is not notified when a drop down window has opened. There is no alert letting them know of the selection made and no notification the drop down window has closed.
- There is no confirmation the repeat sections were engaged and the section was repeated or deleted.
- In the Task function, the icon or action to open the workflow document is not recognized so the user cannot open a workflow document to edit or review.
The Project Management Office roadmap includes future plans to upgrade the Frevvo Mobile Web Forms Electronic Signatures and Approval Workflows in future targeted releases. These upgrades should resolve currently known inaccessible features and functionality limits.
