    Thesis Forms Checklist

    Forms Filled Out Prior to Defense

    • Appointment of Supervisory Committee —Your department must fill out and submit this form to Graduate Admission and Degree Services (located on the third floor of Riverfront Hall, room 307) as soon as is reasonable, no later than the time of submission of the Application for Admission to Candidacy.
    • Application for Admission to Candidacy —Fill out and submit this form to Graduate Admission and Degree Services (Riverfront Hall, Room 307) as soon as you complete half your total credit requirements (generally the second semester of the first year). See the Academic Calendar for the exact deadline date.
    • Graduate Defense Notification – Submit this online form to let the Graduate College know that your graduate defense has been scheduled (announcement is required and should proceed the presentation by at least two weeks).
    • Defense Committee Approval (thesis)—Follow the format provided in this template exactly, ensuring that all names are correct. Print one copy on standard white paper to bring to your defense.  After you have successfully defended, ensure that your advisor and committee members sign and date the page (using blue or black ink and using the date of the defense). Bring your Defense Committee Approval page to the Thesis and Dissertation Office (Riverfront Hall, Room 307) as soon as the page has been signed.
    • Final Reading Approval (thesis) —Follow the format provided in this template exactly, ensuring that all names are correct.  Print one copy on standard white paper to bring to your defense to give to your advisor.  Your advisor will keep the form until after you have incorporated any changes the committee requested during your defense.

    Forms to Submit at the Time You Upload Your Advisor-Approved Version of the Thesis

    • Final Reading Approval (thesis) — After you have incorporated any modifications required by the defense committee, you will submit the revised thesis to the chair of the committee (your major advisor) for a final reading approval.  Once your advisor has signed the Final Reading Approval page, bring the signed copy to the Thesis and Dissertation Office located on the third floor of Riverfront Hall, Room 307.
    • Access Agreement for a Thesis or Dissertation —This form provides you with the opportunity to dictate how you would like your electronic thesis accessed in ScholarWorks. It must be signed by both you and your advisor. Bring the signed copy to the Thesis and Dissertation Office, Riverfront Hall, Room 307.  This information will also be used when you upload your thesis to ScholarWorks.
    • Approval Page for Electronic Copy (thesis) — Follow the template exactly and ensure that all names, titles, etc. are correct.  Include this page in the electronic version of your thesis, which will be uploaded to ScholarWorks.

    Forms Filled Out Prior to Defense

    • Graduate Defense Notification — Submit this online form to let the Graduate College know that your graduate defense has been scheduled (announcement is required and should proceed the presentation by at least two weeks).
    • Defense Committee Approval (dissertation)—Follow the format provided in this template exactly, ensuring that all names are correct. Print one copy on standard white paper to bring to your defense.  Give this form to the Graduate Faculty Representative (GFR) prior to the start of the defense.  The GFR will bring this signed copy to the Thesis and Dissertation Office for safekeeping.
    • Final Reading Approval (dissertation) — Follow the format provided in this template exactly, ensuring all names are correct.  Print one copy on standard white paper to bring to your defense.  Give this form to the Graduate Faculty Representative (GFR) prior to the start of the defense.  The GFR will give the Final Reading Approval to the chair of your committee to be signed after you have incorporated any changes the committee requested during your defense.

    Forms to Submit at the Time You Upload the Advisor-Approved Copy of Your Dissertation

    • Final Reading Approval (dissertation) — After you have incorporated any modifications required by the defense committee, you will submit the revised thesis to the chair of the committee (your major advisor) for a final reading approval.  Once your advisor has signed the Final Reading Approval page, bring the signed copy to the Thesis and Dissertation Office located on the third floor of Riverfront Hall, Room 307.
    • Access Agreement for a Thesis or Dissertation —This form provides you with the opportunity to dictate how you would like your electronic dissertation accessed in ScholarWorks.  Both you and your advisor must sign it.  Bring the signed copy to the Thesis and Dissertation Office, Riverfront Hall, Room 307. This information will also be used when you upload your dissertation to ScholarWorks.
    • Approval Page for Electronic Copy (dissertation)—Follow the template exactly and ensure that all names, titles, etc. are correct.  Include this page in the electronic version of your dissertation, which will be uploaded to ScholarWorks. This page electronically replaces the Defense Committee Approval page and Final Reading Approval page in the electronic copy of the dissertation.
    • Survey of Earned Doctorates (SED) — Please complete the electronic registration for the Survey of Earned Doctorates: https://sed.norc.org/doctorate/ This form is submitted to the National Organization for Research at the University of Chicago and will be published in a Summary Report detailing all of the doctorates awarded in the United States.
